1. Are systematic reviews of design and/or development conducted at suitable stages?
2. Do these reviews:
a. Evaluate the ability to fulfill requirements?
b. Identify problems and propose follow-up actions?
3. Do participants in such reviews include representatives of functions concerned with the design and/or development stage(s) being reviewed?
4. Are the results of the reviews and subsequent follow-up actions recorded?
5. At what stages of the design are plans or other design documented?
6. Where are records of design reviews maintained?
